[Bug][Claude Code] Claude PreToolUse and PostToolUse hooks fail

When using the Claude Code hooks (/hooks), the PreToolUse, PostToolUse, and PostToolBatch hooks fail when triggered.

To reproduce, create a PreToolUse or PostToolUse hook in claude settings directly, or instruct Claude Code to do so.
