feat(workflow-form): add the Form View entry points and turn it on

Task Summary

Wire the reachable entry points and turn the feature on.

• flip form-view-enabled to true
• app-routing.module.ts: route workflow/:id/parameters → WorkflowFormComponent
• dashboard list-item: toggle + Form View icon (applyParameterizedState, gated on formViewEnabled && isParameterized)
• canvas menu: view-switch to the form
